Permanent permission to stay for the term of five years
A police department issues a permanent stay permit for the period of five years if there are no grounds to refuse a visa application of the citizen of a third country:,- who is the spouse of the citizen of the third country, who has a permanent stay permit on the territory of Slovak Republic or is a dependent relative of direct descent of a citizen of Slovak Republic, and who has permanent residence in Slovak Republic,
- who is an unmarried child under 18 of a citizen of a third country, who is the spouse of a citizen of Slovak Republic, who has permanent residence in Slovak Republic,
- who is an unmarried child under 18 of a citizen of a third country, who has a stay permit for the period of five years or is a child under 18 under wardship of a citizen of a third country who has a stay permit for 5 years,
- who is a child of a person of a third country who has a permanent stay permit, has no means of subsistence, is aged over 18 requiring personal care for health reasons,
- if it is in the national interest of Slovak Republic.
DOCUMENTS TO BE ENCLOSED TO THE APPLICATION:
1) 2x photos sized 3x 3.5 cm,
2) valid international passport,
3) administration fee payment receipt to the sum of 4,5 Euro,

- extract from the criminal record of the applicant’s citizenship-country, translated into the Slovak language and notarized or certified by a translation agency (or a legal translator
- extract from the criminal record of the country where the applicant has stayed within the last three years for the period exceeding 90 days during 6 months in succession.
- extract from a bank account opened in the name of a citizen of a third person to the threshold amount of twelve living-wages (annually 2,277.96 Euro). For under age citizens of a third country - a half of this threshold amount (annually -1138.9 Euro) must be confirmed.,
- a letter from the employer indicating the agreed wage rate,
- a work contract and the employer’s letter indicating the factual wage rate,
- extract from a bank account or a document certifying the pension received by the husband, a parent or a citizen of Slovak Republic, who is the relative of the first relationship line of a citizen of a third country, and his written consent to provide financial and material sponsorship to the citizen of a third country to the amount not less than twelve living-wages annually 2,277.96 Euro). For under age citizens of a third country a half of the threshold amount (annual amount - 1138.9 Euro) must be confirmed.
- the accommodation must be shared with a citizen of Slovak Republic or a citizen of a third country who has a permanent residence for the period of five years.

The foreign citizen of a third country must submit to a foreign affairs police department a medical certificate which attests that he has no dangerous diseases. Such medical certificate is valid within 30 days from the date of its issue.
